APPOINTMENT INFORMATION

PHONE APPOINTMENTS

​

  • Please be ready 15 minutes prior to your appointment time. 

  • Please allow a one hour window time-frame for your appointment. (In case your doctor is running behind and unable to call at the scheduled appointment time.)

  • Confirm that your phone is near and turned on, with the volume up.

  • Doctor's will likely call from a private or blocked number.  Please be sure your phone can accept calls from private or blocked numbers.

  • If you miss your appointment phone call the doctor will try calling you again.

IN-OFFICE APPOINTMENTS

  • Street parking only (time limits apply).  Please be sure to leave enough time to find street parking as it available to all of the public and we do not have dedicated clinic parking.

  • ​​Arrive at our office as close to your scheduled appointment time as possible - no earlier than 5 minutes ahead.

  • CHECK IN AT OUR SELF SERVCE KIOSK at the front door with your BC Services Care Card. Scan the bottom rectangular QR Code on the back of your card.

  • Do not bring extra people with you unless medically necessary.

BOOK AN APPOINTMENT

Patients already registered with our Patient Portal can SIGN IN HERE to make an appointment online.

All other patients can call our office to book an appointment or to receive your access code to register for the  Patient Portal online. 

PRESCRIPTION RENEWALS

​Please note that in order to get your prescription renewed you will need to book an appointment with your doctor.  After speaking to your doctor, it may take up to 24 hours for your prescription to be faxed to your pharmacy.  Your patience is greatly appreciated.

  • If you require the doctor to complete a form, we ask that you send the form PRIOR to your appointment. 

  • Click here to email the form or drop it off in-person at our office.​

  • We require you to complete your portion of the form prior to sending to us.

NOTES + LETTERS

 

  • Charges will apply for all note and letter requests.

  • Charges vary.  We follow the suggested pricing guidelines provided by BC Family Doctors.

Cancellation Policy

CANCELLATION POLICY

Our cancellation policy requires patients to cancel their appointments AT LEAST 24 HOURS BEFORE the scheduled appointment time to avoid a $93 Cancellation Fee.  ​

 

Missed phone or in-office appointments and late cancellations (appointments cancelled within 24 hours) will be subject to the cancellation fee.

​

Late arrivals may not be seen by their doctors if their scheduling will not allow it.  If so, our cancellation fee will also apply.

Our cancellation policy allows us to better serve all patients.  Thank you for your cooperation and your understanding.

CALL TO CANCEL

You can call the office at 604-926-1510 to cancel your appointment.  You are able to leave a message to cancel your appointment if your calling prior to 24 hours.  If you are calling for a same day cancellation, please do not leave a message and speak directly to our team.

PORTAL CANCELLATIONS

If you have already registered and have access to our patient portal, you will be able to cancel your appointment online only up until 48 hours prior to your appointment.  If you need to cancel after the 48 hour mark you will need to call our office directly.​​​

​If you have an outstanding cancellation fee, you are able to pay online here.

PATIENT PORTAL

All patients are invited to join our Patient Portal.​  Our patient portal allows you to:

​

  • Schedule your appointments online

  • Confirm or cancel existing appointments.

  • Receive communication from the AMC Team.
